HRET HIIN is excited to partner with 6 CMS designated Quality Improvement Network-Quality Improvement Organizations. As part of our HIIN, our QIN-QIO partners will bring participating hospitals together with other health care providers such as nursing homes, home health agencies, hospices and local organizations to accomplish four major goals: promote effective prevention and treatment of chronic disease by promoting safe care that is patient and family-centered, reliable and accessible; make care safer and reducing harm caused in the delivery of care; promote effective communication and coordination of care; and make care more affordable.
The Atom Alliance QIN-QIO works with health care providers in Alabama, Indiana, Kentucky, Mississippi, and Tennessee.
The Great Plains QIN-QIO works with health care providers in Kansas, Nebraska, North Dakota and South Dakota.
The Health Quality Innovators QIN-QIO works with health care providers in Maryland and Virginia.
The Mountain-Pacific QIN-QIO works with health care providers in Montana, Wyoming, Hawaii, Alaska and the U.S. Pacific Territories of Guam and American Samoa and the Commonwealth of the Northern Mariana Islands.
As part of the New England QIN-QIO, Healthcentric Advisors works with health care advisors in Massachusetts, Maine and Rhode Island while subcontractor, Qualidigm, works with those in Connecticut, New Hampshire and Vermont.
